Fadi HashimyEndodonzia
Complex anatomy. Hidden connections. Controlled treatment. This case documents the endodontic management of a mandibular second molar presenting with a C-shaped root canal system — one of the most anatomically demanding configurations encountered in clinical endodontics. Mandibular second molar with a C-shaped root canal system. Two distinct orifices (mesial and distal) are connected by a narrow groove and fin visible under magnification. Anatomical Presentation The tooth exhibited two distinct canal orifices — mesial and distal — connected by a narrow groove and fin forming the characteristic C-shaped configuration. This interconnecting fin represents one of the primary clinical challenges: standard rotary instrumentation can reliably shape the main canals, but the fin itself is largely inaccessible to mechanical files.
