A patient was referred to me for the retreatment of tooth 2.6 Pre op X-ray Single tooth isolation Removal of the existing restoration and decayed tissue I used liquid dam to improve isolation I removed the existing filling material and found the MB2 canal, confluent with the MB1 Cone fit Despite the apex locator indicating the correct WL, the GP in the mesial canal was long, as often can happen in the teeth in communication with the maxillary sinus While I was irrigating, the patient inhaled and the sodium hypochlorite disappeared from the buccal canals. The patient suddenly felt intense pain and brought his hands to the sinus area. I Immediately stopped irrigating and washed several times with sterile water…
