The patient came with severe pain in the lower right quadrant, associated with teeth #46 and #45. Both teeth were diagnosed with irreversible pulpitis and showed extensive caries together with overgrown gingival tissue. Despite the challenging scenario, the main objective was clear: preserving the natural dentition through a conservative and sequential approach. Field control was established using a split-dam isolation, and pre-wedging facilitated a conservative gingivectomy to remove only the excessive papilla while maintaining soft tissue harmony. Once the surgical field was secured, attention was directed to complete caries removal—an essential step that should never be overlooked before initiating any access cavity.
