Complex Retreatment of Over-Extended Carrier-Based Obturation Using Retreaty Kit with Bioceramic Sealer
Riccardo Tonini
Carrier-based obturation systems such as Thermafil offer efficient three-dimensional filling of the root canal system, yet they carry a well-documented risk of extrusion beyond the apex when working-length control is suboptimal. Retrieval of an over-extended plastic carrier represents one of the more demanding challenges in non-surgical endodontic retreatment, requiring dedicated instrumentation and a clear clinical strategy. This case documents the step-by-step retreatment of a tooth with an over-extended carrier-based obturation, the removal accomplished with the Retreaty Kit (Perfect Endo) , and re-obturation with a bioceramic sealer and single-cone technique , with radiographic follow-up at six months and one year confirming periapical healing. Pre-Operative Assessment The patient presented with persistent periapical symptoms associated with a previously treated tooth. Clinical examination revealed…
