Abstract Successful endodontic treatment depends on proper diagnosis, thorough canal debridement, and three-dimensional obturation. This article describes the clinical management of a posterior tooth presenting with slight mobility and occlusal pain, complicated by narrow mesial canals and the presence of old amalgam restorations. A modern protocol incorporating ultrasonic activation, EDTA irrigation, and bioceramic obturation was used to achieve predictable results. Introduction Teeth presenting with occlusal trauma, caries, and previous restorations often require a carefully modified endodontic approach. Narrow canals, especially in mesial roots, increase the risk of procedural errors and incomplete cleaning. Modern irrigation protocols, ultrasonic activation, and bioceramic sealers have significantly improved disinfection and long-term sealing of the root canal system.
